Requirements for Enrollment
New students must fulfill the Health Services requirement for entry to the College once during enrollment by completing a health certificate form (PDF).
Sections I through V should be filled out by students. Health care providers should complete sections VI through VII after a physical exam.
Illinois state law requires that all students must provide proof of receiving two measles, one mumps, and one rubella immunization after age one, as well as one tetanus/diphtheria booster within the last ten years. MacMurray College also requires a one-step TB skin test to be completed with documentation that it has been read and the results. Students who intend to major in nursing or education will need the two-step TB skin test to be completed with documentation that it has been read and the results.
The American College Health Association (ACHA) now recommends that students entering college be immunized for Meningococcal Meningitis and Hepatitis B. It is also recommended that students consult with their health care provider regarding the need for the chicken pox vaccine if they have not had chicken pox.
Completed health forms should be brought to registration or may be sent to the Office of Student Life prior to registration.
