Requirements for Enrollment
As you approach enrollment in MacMurray College I wish to bring to your attention the Health Services' requirement for entry to the college. You will be asked to fulfill these requirements only once during your stay at MacMurray, so it is expected that you complete them as soon as possible.

You need to complete sections I through V. You are then required to have a physical exam by your health care provider. He/she will complete sections VI-VII.
Please note that Section VII should be completed by your health care provider. Illinois state law requires that all students must provide proof of receiving two measles, one mumps, and one rubella immunization after age one, as well as one tetanus/diphtheria booster within the last ten years. MacMurray College also requires a one step TB skin test to be completed with documentation that it has been read and the results. If you are going to be a nursing or education major, you will need the two step TB skin test to be completed with documentation that it has been read and the results, so you might as well get the two step TB rather than the one step.
The American College Health Association (ACHA) now recommends that students entering college be immunized for Meningococcal Meningitis and Hepatitis B. It is also recommended that you consult with your health care provider regarding the need for the chicken pox vaccine if you have not had chicken pox.
It is important to remember to take this health certificate form and your immunization records with you at the time of your physical, so the physician can determine if any shots are needed. Please remember that having met all the requirements will assure that your registration is not delayed. You may send your completed health form prior to registration to the Student Life Office.
